Required Documentation for all Physical Therapist Candidates:

 

      Resume – Listing duties, awards, Pt. Diagnoses, Date resume created, contact info, 3 desired employment locations (mandatory)

      College Diploma (mandatory)

      Current Foreign Physical Therapy License or Foreign Physical Therapy Assistant License (mandatory)

       U.S. Physical Therapy Assistant License – You must have already obtained your license to apply.

       College Transcripts (mandatory)

       Passport

       Birth Certificate (mandatory)

       Spouse’s Birth Certificate, Passport, and visa (if married)

       Marriage Certificate. If either spouse was previously married you must provide a photocopy any divorce or death certificates. (mandatory if applicable)

      Each child’s Birth Certificate, Passport and visa (mandatory if applicable)

      ICHP/FCCPT Credential Evaluation (mandatory when obtained)

      English language test results letter – TOEFL iBT

 

PLEASE NOTE INTERNSHIP DOES NOT COUNT AS EXPERIENCE.  Only list any experience you have obtained as a licensed PT on your resume.
