Required Documentation for all PT Candidates:
- Resume - Listing duties, awards, Pt. Diagnoses, Date resume created, contact info, 3 desired employment locations (mandatory)
- Photocopy of College Diploma (mandatory)
- Photocopy of current Foreign PT License (mandatory)
- College Transcripts, original in sealed envelope by school (mandatory)
- Photocopy of Passport and visa (mandatory)
- Photocopy of Birth Certificate (mandatory) NSO Copy needed within 3 months of filing.
- Photocopy of spouse’s Birth Certificate NSO Copy needed within 3 months of filing., Passport, and visa (if married)
- Photocopy of your Marriage Certificate. NSO Copy needed within 3 months of filing. If either spouse was previously married you must provide a photocopy any divorce or death certificates. (mandatory if applicable)
- Photocopy of each child’s Birth Certificate NSO Copy needed within 3 months of filing., Passport and visa (mandatory if applicable)
- TOEFL IBT passing score (mandatory)
- Visa Screen Certificate
- 8 passport photos (mandatory - if in the USA immigration style photos 2x2)
- 3 Signed Signature pages (PAGE 8) of the ETA 9089 Form
- Email address (mandatory)
- PT Board Approved to sit for NPTE
- Employment Certifications (mandatory)
